Before completing this form, locate the course you want to attend using the list below.  Make a note of the date you wish to attend.  You may submit the form more than once if you wish to submit multiple registrations.

After completing the form, press the SUBMIT button. At that time, your information will be forwarded to CFS and a Confirmation form will be displayed. To complete your enrollment you have two options:

1.Click the provided link on the confirmation page to pay securely using your credit card or Paypal account. Print the form for your records; you do not need to mail anything to us, or

2. Print that form, attach a check with your registration fee from the list below and mail it to the address indicated on the feedback form you will see after pressing submit.
